چکیده

Background: Congenital malformations of the auricle are often combined with atresia of the outer ear canal and malformations of the ossicles, representing aesthetic as well as functional deficits. Optimal treatment should therefore address both aspects equally. This report describes a new approach, combining the reconstruction of the auricle with implantation of an active middle ear hearing aid, stimulating the round window membrane. Method: A 33-year-old male patient, with bilateral ear microtia, fibrous atresia of the external ear canals and malformation of the ossicles due to Treacher Collins-Franceschetti syndrome was included in the study. In stage one, the cartilage framework of the new auricle, made of autogenous rib cartilage, was fabricated and implanted. During stage two, the auricle was elevated, a retro-auricular sulcus was formed and a Vibrant MED-EL Soundbridge device was implanted. The transducer was coupled to the round window membrane. Results: Both functional and aesthetical results were favourable. Aided thresholds were between 15 and 30 dB in the frequency range of 0.75–6 kHz, monosyllabic word understanding at Published online: October 26, 2006 PD Dr. Jan Kiefer Klinik und Poliklinik für HNO-Heilkunde Klinikum r.d. Nonlinear time-domain cochlear model for transient stimulation and human otoacoustic emission.

This paper describes the implementation and performance of a nonlinear time-domain model of the cochlea for transient stimulation and human otoacoustic emission generation. The nonlinearity simulates compressive growth of measured basilar-membrane impulse responses. The model accounts for reflection and distortion-source otoacoustic emissions (OAEs) and simulates spontaneous OAEs through manipu...

Retrograde Waves in the Cochlea

Retrograde waves in the cochlea are important because they provide information about cochlear mechanics that may be measured in the external ear canal as otoacoustic emissions. By extending traditional wave-variable methods, an explicit equation is derived for the forward-traveling wave in a one-dimensional cochlear model that is equivalent to the WKB approximation. The corresponding equation f...

Measurement of Amplitude and Delay of Stimulus Frequency Otoacoustic Emissions.

Although stimulus frequency otoacoustic emissions (SFOAEs) have been used as a non-invasive measure of cochlear mechanics, clinical and experimental application of SFOAEs has been limited by difficulties in accurately deriving quantitative information from sound pressure measured in the ear canal. In this study, a novel signal processing method for multicomponent analysis (MCA) was used to meas...

Tinnitus after head injury: evidence from otoacoustic emissions.

OBJECTIVE Tinnitus may be caused by a lesion or dysfunction at any level of the auditory system. This study explores cochlear mechanics using otoacoustic emissions in patients with tinnitus after head injury, in whom there seems to be evidence to support dysfunction within the CNS. METHODS The study included 20 patients with tinnitus and other auditory symptoms, such as hyperacusis and diffic...

Growth of Distortion-product Otoacoustic Emissions in a Nonlinear, Active Model of Cochlear Mechanics

Evidence of compressive growth of basilar membrane displacement can be observed in measurements of DPOAE. When the levels of the two stimulus tones are related by the formula L1=0.4 L2+39, then the DPOAE I/O function resembles the classic Fletcher-Munson (1933) loudness function. In order to understand how the DPOAE I/O function relates to the perception of loudness, we need to also understand ...

The origin of periodicity in the spectrum of evoked otoacoustic emissions.

Current models of evoked otoacoustic emissions explain the striking periodicity in their frequency spectra by suggesting that it originates through the reflection of forward-traveling waves by a corresponding spatial corrugation in the mechanics of the cochlea. Although measurements of primate cochlear anatomy find no such corrugation, they do indicate a considerable irregularity in the arrange...
